2. TERMS & CONDITIONS
By using the Synergy Diagnostic Lab website or scheduling and receiving our services, you agree to these Terms & Conditions.
Services
Synergy Diagnostic Lab provides professional specimen collection services, including in-office and mobile phlebotomy services and other specimen collection services offered by the company.
Services may include, where applicable:
- Routine blood draws
- Mobile blood draws
- Specialty laboratory kit collection
- DOT specimen collection
- Non-DOT specimen collection
- Rapid testing services
- Employer and group collection services
Scope of Services
Synergy Diagnostic Lab is primarily a specimen collection service unless otherwise specifically stated.
Collection of a specimen does not necessarily mean Synergy Diagnostic Lab performs the laboratory analysis. Specimens may be transported or submitted to an appropriate laboratory or testing facility for analysis.
Client Responsibilities
Clients are responsible for:
- Providing accurate information
- Providing required identification when applicable
- Providing required laboratory orders, requisitions, or test kits
- Following preparation instructions provided by the ordering provider or laboratory
- Informing Synergy of relevant circumstances that may affect collection
- Providing a safe environment for mobile appointments
Appointment Availability
Appointments are subject to availability.
An appointment is considered scheduled when Synergy confirms the appointment through its designated scheduling process.
Right to Refuse or Stop Service
Synergy Diagnostic Lab reserves the right to refuse, postpone, or discontinue a service when:
- The requested service cannot safely be performed
- The client or location presents a safety concern
- Required documentation is unavailable
- The requested service is outside Synergy's scope
- The service cannot be performed according to applicable requirements
- A client or third party behaves in a threatening, abusive, or inappropriate manner
No Medical Diagnosis
Synergy Diagnostic Lab does not diagnose medical conditions or interpret laboratory results unless specifically authorized and qualified to do so.
Questions regarding medical conditions, laboratory results, diagnosis, or treatment should be directed to the client's healthcare provider.
3. CANCELLATION & RESCHEDULING POLICY
We understand that plans can change. We ask that clients provide as much notice as possible when canceling or rescheduling an appointment.
Cancellation
Clients should provide at least 24 hours' notice when canceling or rescheduling an appointment.
Appointments canceled with less than 24 hours' notice may be subject to a cancellation fee.
No-Show Appointments
A "no-show" occurs when a client is unavailable or fails to appear for a scheduled appointment without providing notice.
A cancellation or no-show fee may apply.
Mobile Appointments
For mobile appointments, the client must be available at the agreed location during the scheduled appointment window.
If the phlebotomist arrives and is unable to complete the appointment because the client is unavailable, a no-show or travel fee may apply.
Rescheduling
Clients may request to reschedule an appointment. Rescheduling is subject to availability.
Emergencies
Synergy may waive a cancellation fee at its discretion when circumstances outside the client's reasonable control occur.
4. MOBILE SERVICE POLICY
Synergy Diagnostic Lab provides mobile phlebotomy and specimen collection services at approved locations throughout Jacksonville, Florida and surrounding areas.
Mobile Appointments
Mobile services must be scheduled in advance unless same-day or urgent appointments are specifically available.
The client must provide:
- Accurate service address
- Contact telephone number
- Appointment information
- Any required laboratory order or test kit
- Safe access to the appointment location
Safe Environment
Clients must provide a safe, clean, and reasonably accessible area for specimen collection.
Synergy may decline or discontinue a mobile appointment if conditions create an unsafe environment for the client or phlebotomist.
Travel & Mileage
Mobile blood draws are subject to the applicable mobile service fee.
Mileage or travel fees may apply depending on the client's location.
Any applicable travel fee should be communicated to the client before the appointment whenever reasonably possible.
Pets
For the safety of the client and phlebotomist, pets may need to be secured away from the collection area during the appointment.
Additional People
A parent, guardian, caregiver, or authorized support person may be present when appropriate.
Synergy may limit additional persons in the collection area when necessary for privacy, safety, or specimen-collection requirements.
Weather and Unsafe Conditions
Mobile appointments may be delayed, rescheduled, or canceled when severe weather, unsafe road conditions, or other circumstances create a safety concern.
5. PAYMENT POLICY
Payment is due at the time of service, unless a separate written agreement has been established with a business, facility, laboratory, healthcare provider, or other authorized organization.
Accepted Payment Methods
Synergy Diagnostic Lab accepts the payment methods identified at the time of booking or service.
These may include:
- Major credit cards
- Debit cards
- Cash
- Other payment methods specifically made available by Synergy
Service Fees
Current service prices are listed on our website or communicated during scheduling.
Mobile travel or mileage fees may apply depending on location.
Business Accounts
Businesses and organizations with an approved account or written service agreement may be subject to separate payment terms.
Refunds
Refund requests are reviewed on a case-by-case basis.
Because specimen collection services involve staff time, supplies, travel, and other expenses, completed services may not be refundable.
Payment Processing Fees
If applicable, payment processing fees may be disclosed to the client before payment.
6. HIPAA & PRIVACY-RELATED INFORMATION
HIPAA Privacy Statement
Synergy Diagnostic Lab is committed to protecting the confidentiality and security of personal and health-related information.
HIPAA applies to certain healthcare providers, health plans, healthcare clearinghouses, and business associates. Whether HIPAA applies to Synergy for a particular service depends on the nature of the service and Synergy's role in the applicable healthcare transaction.
When HIPAA applies to Synergy's activities, Synergy will handle protected health information in accordance with applicable HIPAA requirements and applicable agreements.
Where Synergy operates as a business associate of a HIPAA covered entity, applicable business associate agreements may govern the permitted uses and disclosures of protected health information. HHS explains that business associates may have contractual and direct obligations concerning protected health information.

Synergy Diagnostic Lab LLC provides specimen collection services and is not a substitute for a physician, healthcare provider, emergency medical service, or diagnostic laboratory unless specifically identified as providing that service.
Our phlebotomy services generally involve collecting blood or other specimens requested by an authorized healthcare provider, laboratory, employer, or other authorized party.
Synergy Diagnostic Lab does not independently diagnose medical conditions or prescribe, recommend, or change medical treatment based solely on specimen collection.
Laboratory testing and interpretation of results may be performed by an independent or third-party laboratory.
Clients should direct questions regarding:
- Medical diagnosis
- Laboratory results
- Treatment
- Medication
- Medical conditions
- Test interpretation
to their healthcare provider or the laboratory responsible for testing.
Emergency Medical Situations
Synergy Diagnostic Lab's services are not intended for medical emergencies.
If you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Test Orders
Clients may be required to provide a valid laboratory order, requisition, test kit, or other documentation depending on the requested service.
Synergy reserves the right to decline collection when required documentation is unavailable or when the requested collection cannot be safely or appropriately performed.
Laboratory Results
Synergy Diagnostic Lab does not guarantee a particular laboratory result or medical outcome.
Testing, turnaround times, and results are dependent upon the laboratory or testing facility performing the analysis and other factors outside Synergy's control.
